Exercise Referral

Perfect fit is part of the PROACTIVE exercise referral scheme run by Somerset Primary Care Trust

Have you been told by your doctor that you need to take more exercise? Do you have a medical condition that is listed below? This programme is followed with your doctor or health professional’s consent and follows strict guidelines to ensure that the exercise programme you follow is safe as well as effective.

Do you have the following conditions?

  • Diabetes, Hypertension or Angina
  • Osteoporosis
  • Depression or Anxiety
  • Obesity (BMI > 30) or Body Fat percentage 32+ for women and 25+ for men
  • Back Pain

how can exercise help?

Physiological and biomechanical changes occur in the body following exercise, for example, recent evidence suggests that middle –aged or older men who take up regular, moderate intensity physical activity/exercise have a 23% reduced risk of a heart attack compared to their counterparts who do not take up regular exercise/activity. Regular exercise can help reduce symptoms of these diseases, and in some cases reverse the condition.
Individualised programmes are devised with the consent of your health professional and will follow the National Quality Assurance Framework for Exercise Referral.

If you wish to join the Exercise referral scheme just ask your doctor or health professional to be put on the Proactive scheme. You can then access an appropriate 12 session exercise programme.
